Home Health Certified Occupational Therapist AssistantFull-Time | Field-Based

What You’ll Do:Provide occupational therapy services to patients in their homes under the supervision of an Occupational Therapist
Follow and implement individualized plans of care to support patient independence and safety
Monitor patient progress and communicate changes in condition to the OT, physician, and care team
Educate patients and caregivers on therapy goals and home exercise programs
Maintain timely, accurate clinical documentation
Collaborate closely with interdisciplinary team members to ensure high-quality care
Help reduce avoidable hospitalizations and ER visits through proactive, patient-centered care

Graduate of an accredited occupational therapy assistant program
Active, unrestricted COTA certification in the state of practice
Minimum 1 year of clinical experience as a COTA
Comfortable working independently in a field-based role
Ability and willingness to travel within the assigned coverage area (approximately 50%)
Reliable vehicle, valid driver’s license, and current auto insurance
Ability to lift up to 50–100 lbs and perform physical tasks associated with patient care
