Qualifications:

• MD/DO from an accredited medical school
• Accredited Urology Residency Program completion
• American Board Eligible or American Board Certified in Urology
• Ability to obtain an unrestricted Pennsylvania Medical License and ability to obtain necessary credentials
• DEA certification
