We are seeking an experienced Registered Nurse (Healthcare Coordinator) for a well-established, growing company. This opportunity requires relocation to Gillette, Wyoming. The ideal candidate for the Registered Nurse (Health Coordinator) position will be responsible for improving patient outcomes by identifying and closing gaps in care.

Registered Nurse (Healthcare Coordinator) Qualifications and Responsibilities:

• Active Registered Nurse (RN) or Practical Nurse (LPN) license in Wyoming, or be eligible 

• Relevant certifications (case management, care coordination, patient navigation) are a plus

• 2+ years of clinical experience in a hospital, outpatient clinic, or ambulatory care setting

• Experience in care coordination/case management/population health or preventive care

• Prior experience using EMR systems regularly, and pulling reports

• Coordinate and schedule appointments/services, following up to ensure care is completed

• Document all interactions and referrals in EMR; maintain accurate records for continuity of care

Registered Nurse (Healthcare Coordinator) Compensation & Benefits:

• Salary range of $70K - $90K, based on experience
