Job Summary:
We are seeking a compassionate and skilled Physical Therapist (PT) to provide patient-centered care in skilled nursing and post-acute settings. The PT will evaluate and treat residents, develop individualized treatment plans, and collaborate with an interdisciplinary team to ensure coordinated care. Accurate documentation and participation in care conferences and quality improvement initiatives are essential. This role offers flexibility with full-time or travel/contract options.
Responsibilities:
- Evaluate and treat residents in skilled nursing and post-acute care
- Develop and implement individualized treatment plans to optimize functional outcomes
- Collaborate with nursing, therapy, and medical staff for seamless care
- Document care accurately and timely per facility and regulatory standards
- Participate in care conferences and quality improvement initiatives
Qualifications:
- Active California Physical Therapist license required
- Minimum 2 years of relevant experience preferred
- Skilled nursing or post-acute care experience preferred
- Strong clinical, communication, and teamwork skills
- BLS/BCLS certification required
