Location:Remote, Cedar Rapids, IA (Candidates should live close to the targeted city and may occasionally be asked to visit patients' homes for specific situations, though this is rare.)
Specialty: Nurse Practitioner/Family Medicine
Required Certifications:
- Board Certified
- State licensure in IA
Previous Experience Desired:
- 2 years of direct patient care experience related to polychronic conditions
Start Date: ASAP
Duration/Schedule: 3 months with likely extensions
On-call/Callback: No call required
Types of Cases/Procedures:
- Maintain quality of care standards
- Assist patients with enrolling in educational programs
- Knowledge of disease diagnosis and prevention
- Assess patients' health status
- Implement and follow up on care plans
- Document patient information accurately and timely
Patient Volume: Complete 3+ patient visits daily
Additional Notes:
- This role primarily involves virtual work with occasional travel within a specific geography for in-home assessments.
- Training includes 3 days onsite and the remainder virtually.
- Monday to Friday, 8am-5pm in the candidate's time zone.
- Collaborating physician available for support.
