What You ll Do

• Provide comprehensive outpatient primary care in a value-based care model
  

• Care for an established 65+ patient population

• Collaborate within a multidisciplinary, team-based environment

• No inpatient responsibilities

Practice Highlights

• 100% outpatient, Monday Friday schedule

• One work-from-home day per week
  

• Approximately 18 patients per day

• Limited call responsibilities

• Robust team-based support including EMTs, APPs, Medical Assistants, Care Managers, and acute care nursing support
