Minimum Requirements

  • Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
  • Active Indiana Physical Therapist license required to start the assignment.
  • BLS certification from AHA or ARC may be required.
  • Experience working in a skilled nursing facility or long-term care setting preferred.
  • Clinical experience treating geriatric patients and managing post-acute rehabilitation preferred.
  • Ability to document in electronic medical record systems preferred.
