Job Description & Requirements

Part-Time | 2–3 Days per Week | 8:30 AM – 5:00 PM
On-Site / Community-Based Role

• Provide nursing care to PACE participants in their homes, community, and clinic settings
• Conduct comprehensive nursing assessments and triage participant needs
• Collaborate with an interdisciplinary care team to create and manage individualized care plans
• Educate participants and families on chronic and acute health conditions
• Communicate changes in participant condition and coordinate transitions of care
• Maintain accurate electronic documentation and clinical records

• Participate in quality improvement initiatives
