Home Health Primary Care Opportunity in Chattanooga, Tennessee
Lead a collaborative care team and deliver personalized medicine directly to patients in their homes. - You'll lead a Chattanooga-based team of 2 APPs and manage a personal panel of 140 patients
- 100% home-based care with no clinic or hospital responsibilities
- Blend of chronic disease management and coordinated team care in patients' homes
- Serve as clinical lead: mentoring, chart review, oversight, and quality improvement
- Monday-Friday only; no weekends or after-hours requirements
- Half of the week is dedicated to patient visits; the other half to leadership/admin duties
- 6-8 home visits per day, each lasting 30-40 minutes
