Responsibilities and Duties
- Travel to long-term care facilities and patient residences to deliver hands-on wound care (approximately 90 percent in LTC settings)
- Utilize advanced wound care techniques, including UltraMist Wands and application of skin grafts (fish skin, placenta, lab-grown)
- Manage wound, ostomy, and continence care for complex cases
- Document patient care and outcomes using the WoundExpert EMR system
- Collaborate with facility staff to ensure continuity and quality of wound care
- Adapt care plans to meet individual patient needs and facility protocols
Additional Information
- Must have current Nurse Practitioner license in Minnesota
- Required certification: Wound, Ostomy and Continence Nursing (WOCN)
- Demonstrated hands-on wound care experience required
- Experience with surgical PAs, wound care, and podiatry highly preferred
- Candidates with advanced wound care experience will be fast tracked
- Familiarity with UltraMist Wands and various skin graft materials is essential
