Job Description
A Santa Barbara based clinic is searching for a Nurse Practitioner who is passionate about improving individual and community health. An ideal candidate speaks Spanish and English and has experience conducting Annual Wellness Visits. This NP should be versatile as it would travel among 3 clinics, may conduct telehealth visits, and in-home visits.
Required Qualifications
- Current California Nurse Practitioner license
- Nurse Practitioner Certification from either the ANCC or AANP
- Current DEA licensure
- Active CPR Certification
- 2+ years of NP experience (home care and geriatrics are a plus)
Responsibilities
- Conduct annual wellness visits daily for primary care management of patient caseload
- Manage both acute and chronic conditions through direct intervention like medication prescription and patient education
- Consultation with collaborating Physician as appropriate
- Complete documentation of patient encounters, funding sources, and billing according to policy
