The role requires licensure, DEA certification, and proficiency with EMR systems, supporting community health through direct patient care and coordination of services.

Job Description
A Santa Cruz, California based clinic is searching for a Nurse Practitioner or Physician Assistant who is passionate about improving individual and community health. An ideal candidate is a motivated and experienced provider who is comfortable seeing all ages.
Required Qualifications
- Current California Nurse Practitioner or Physician Assistant license
- Current DEA licensure
- Active CPR Certification
Responsibilities
- Assess and diagnose a variety of diseases and injuries in patients of all ages
- Order diagnostic tests and laboratory studies, interpret results and create treatment plans based on results
- Complete annual wellness exams in office and occasional home visits
- Perform therapeutic procedures with prioritizing preventative care
- Arrange referrals, consultations, and therapeutic services for patients on caseload
- Document all patient encounters efficiently and according to organization policy
