Education Requirements

• Degree in Physical Therapy (DPT or equivalent) from an accredited program

Certifications Requirements

• Active California State Physical Therapist license
• CPR certification

Experience Requirements

• Minimum 1 year of home health experience
• Proven ability to manage 5-6 patient visits daily

Job Responsibilities

• Provide skilled physical therapy assessments and treatments to home health patients
• Develop personalized treatment plans and adjust based on patient progress
• Manage 5-6 patient visits per day within a 30-mile radius of Jackson, CA
• Educate patients and families on mobility, strength, and pain management
• Document patient progress accurately in the EMR system (Matrix)
• Collaborate with healthcare teams to coordinate care
