Position Overview
This position is 100% mobile you ll travel to patients homes and provide care directly in their living environments. This role is ideal for a provider who values autonomy, continuity of care, and building long-term patient relationships.
Key Details
- Setting: Home-based primary and palliative care
- Schedule: Patient visits throughout the week, with 1 2 administrative days for documentation, planning, and case reviews
- Patient Load: Approximately 150 visits per month, with flexibility based on patient needs
- Compensation:$115,000 base salary with additional earning potential for exceeding visit expectations

Qualifications
- Active Georgia license and national certification as an NP or PA
- Experience or strong interest in home-based primary care and/or palliative care
