Will consider locum, locum to perm, and perm providers.

Practice Setting: Neurology clinic

Dates of Coverage: As soon as credentialed

Rate Type:Hourly

Schedule: Monday Friday, 8:00 AM 4:30 PM

Practice Demographics:

• 95% Adult

• 5% Pediatric

Patient Load:

• office/clinic patients per day (7.2 hours per day)

Other Neurologists Present: Yes

Group Composition:

• 3 Physicians

• 0 Advanced Practice Providers

Onsite Facilities:

• Lab

• X-ray

Office Ancillary Staff:

• 1 RN Navigator

• 4 Medical Assistants (MAs)

• 3 Customer Service Representatives (CSRs)

Documentation System/EMR: Epic

Trauma Level: 2

Board Requirements: Board Certified/Board Eligible

State License Preference: Prefer providers with an existing license to expedite the process

Certification Requirement: BLS

Required Neurology Procedures:

• Radiologic Exam Interpretation (Unofficial)

• EMG (Electromyogram)

• Botulinum Toxin Administration (Botox)

Estimated Credentialing Timeframe: 120 days
