Work directly with kids & families in mobile crisis response—flexible 40-hr weeks with home visits and on-call rotation. LMSW/MFT-A clinician delivering individual, family, group therapy, assessments, treatment plans and crisis intervention. Full-time (40 hrs/wk), second shift, mobile/home-based in CT; ongoing/permanent role with multidisciplinary team. - Master’s in social work, counseling or MFT - Valid Connecticut clinical license (LMSW, PC-A, MFT-A); LCSW/LPC/LMFT preferred - Valid driver’s license for in-home visits - Experience with DSM-5 and clinical documentation - Strong communication, organization, team collaboration skills

Position Summary To provide direct services as a part of a multidisciplinary team the master’s prepared clinician will provide direct care including individual (particularly children/adolescents), couple, family and group therapeutic services to patients, including assessments, treatment plans, crisis intervention and discharge planning.

Essential Duties & Responsibilities Direct Patient Contact: Provide individual and family therapy to children and their families

Participates in direct care to assess needs and coordinate services

Maintains daily/weekly scheduled appointments in electronic records systems

Provide therapeutic services to patients and their families

Work in collaboration with multidisciplinary team members including external partners

Completes required documentation and paperwork including biopsychosocial assessments and treatment planning and implementation in a timely manner

Participate in our high risk program

Demonstrates cultural competency and commitment to patients including LGBTQ+ community

Maintain case load dependent on need

Participates in clinical supervision with LCSW

Provides on-call support on a annual rotational basis if required by program

Provide crisis management, support and triage

Minimum Qualifications Master’s degree in social work, counseling, marriage and family therapy

Possession of valid clinical license in Connecticut (LMSW, PC-A, MFT-A)

Valid driver's license a must for in home services

Comfort with using DSM 5 and understanding diagnoses

Must have excellent writing and communication skills, be highly organized

Must be able to work as a part of a team

Preferred Qualifications LCSW, LPC, LMFT preferred

Previous experience providing clinical care is desirable

Experience/knowledge of substance use disorders

Experience/knowledge in working with LGBTQ+

Previous home visiting experience
