Clinic proudly practices team based care embedding health educators in the clinical care teams to support patient education, self-management support, and case management needs. Additionally, we have modified a process used by Kaiser to support our physicians in completing patients preventative health needs. The practice has used an electronic health record since 2005.Work schedules are flexible to support a work life balance.
