MedAdventures is seeking a compassionate and experienced Physical Therapist to join a PACE program in Albuquerque, NM. This role is ideal for a clinician who enjoys working with the geriatric population in a community-based setting, helping participants maintain independence, prevent falls, and improve functional mobility. You’ll be part of an interdisciplinary care team focused on long-term outcomes and quality of life—not productivity quotas.
Responsibilities:
- Evaluate participants’ functional mobility, balance, and strength to develop individualized treatment plans.
- Provide direct therapy services in the PACE center, participants’ homes, and community settings.
- Collaborate with an interdisciplinary team (OT, SLP, nursing, social work, and medical staff) to coordinate care.
- Recommend and train participants on durable medical equipment (DME) and assistive devices.
- Conduct home safety assessments and provide fall-prevention education.
- Document evaluations, progress notes, and care plans in accordance with PACE program requirements.
- Educate participants, caregivers, and staff on mobility techniques and safe transfer methods.
Qualifications:
- Current and active New Mexico Physical Therapist license.
- Minimum 1 year of clinical experience preferred (geriatric or home health experience a plus).
- Excellent communication and teamwork skills.
- Valid driver’s license and reliable transportation for community visits.

Offering a $10k sign on bonus for a perm role. Rate is negotiable.
