About the Position:

We re looking for an energetic and compassionate Nurse Practitioner or Physician Assistant to join our post-acute care team in Gallup, NM. This full-time role offers flexible Monday-to-Friday hours, so you can finally achieve that elusive work-life balance (yes, it exists!).
In this role, you ll provide care for patients transitioning from acute hospital stays to post-acute settings, ensuring their recovery journey is as smooth as possible. You ll collaborate with an interdisciplinary team, flex your clinical muscles, and still have time to explore all the incredible things Gallup has to offer. Oh, and there is some call involved but nothing you can t handle!

Responsibilities:
- Provide high-quality care in a post-acute care setting.
- Assess, diagnose, and manage patients health conditions.
- Collaborate with a multidisciplinary team to develop and implement care plans.
- Participate in on-call duties (but don t worry we ll keep the late-night drama to a minimum).
- Educate patients and families on treatment plans and recovery strategies.
Requirements:
- Current licensure as a Nurse Practitioner or Physician Assistant in New Mexico (or eligibility to obtain one).
- Strong clinical skills and a passion for patient care.
- Ability to thrive in a collaborative environment.
- A sense of humor we take our work seriously, but not ourselves.
