Health Center is seeking a Family Practice physician to work at our clinics.
-The Center is Federally Qualified Health Centers (FQHC) and provides a network of Family Practitioners, Internists, Psychiatrists, Dentists, and Social Workers.
--Empower patients to take an active role in health maintenance and promote wellness behaviors
--Provide appropriate patient follow-up and communicate effectively with patients
--Refer patients to specialty care as necessary
--Provide clinical supervision to mid-level providers and nursing staff
--Participate in quality assurance activities
--Participate in State and National disease collaborative efforts
--Community Health Center sites are certified Level III Medical Homes, with a highly developed and supportive panel management and case management system in place.
--Robust integrated behavioral health program employing Psychiatrists, Behavioral Health Nurse Practitioners and Licensed Clinical Social Workers embedded within its primary care sites.
