Job Description:
- Perform clinical triage and respond to patient inquiries over the phone.
- Coordinate prior authorizations and manage administrative clinical workflows.
- Provide patient education and ensure appropriate follow-up care.
- Support providers in care coordination and quality initiatives.
- Participate in clinic meetings and contribute to team-based care delivery.
- Occasionally assist with direct patient care when needed.
