Primary Care Nurse Practitioner / Physician Assistant
Location: Phoenix, Arizona
Compensation: $130,000 $150,000 per year (DOE)
Schedule: Full-Time

Patient Volume: 20 to 30 Patients per day

Key Responsibilities:
- Manage acute and chronic conditions
- Provide preventive care, wellness visits, and patient education
- Perform assessments, diagnostics, and evidence-based treatment plans
- Collaborate with physicians, MAs, RNs, and care coordination teams
Prerequisites / License & Certification Requirements:
- Active Arizona NP or PA license
- Primary care or outpatient experience preferred
- Passion for compassionate, patient-centered care
