Responsibilities: Patient Evaluation: Review patients history and perform a physical examination, maintaining complete and accurate medical records. Conduct physical evaluation and diagnostic tests to determine diagnosis and provide appropriate treatment and patient care plan. Endocrinology Care: Diagnose and treat patients with a variety of hormonal diseases and conditions related to the endocrine systems. Provide comprehensive, cost-effective diabetes care with the available support of CDE/RD services. Management Recommendations: Recommend suitable management for the patient, including preventive health recommendations and habits. Prescribe medications and formulate ongoing treatment and disease management plans. Patient-Centered Care: Provide relationship-centered care, taking into account the patients psychosocial and physical needs. Collaboration and Communication: Communicate effectively with the clinical team to ensure total delivery of quality care. Share on-call phone answering responsibilities with other Endocrinologists, presently on or about every six weeks rotation basis. Educational Participation: Participate in medical staff educational programs and meetings. Qualifications: Degree: Doctor of Medicine (MD), Doctor of Osteopathy (DO) or foreign equivalent. Residency: Completion of three-year postgraduate medical training from a U.S. accredited residency program in Internal Medicine. Fellowship: Completion of two-year postgraduate medical training from an accredited fellowship program in the area of practice (e.g., Endocrine, Diabetes, and Metabolism Medicine). Experience: Open to both experienced and newly trained providers. Certification: Board Eligible/Board Certified in both Internal Medicine and Endocrinology. License: Must possess current, unrestricted New York State license. Registration: Must possess Drug Enforcement Agency (DEA) registration or be eligible for registration. Enrollment: Must possess Medicare/Medicaid Enrollment or be eligible to enroll. Certification: BLS or ACLS certification required.
