Job Description
American Traveler is seeking a Physical Therapist for a home health role requiring a CT PT license and at least 2 years of experience.
Responsibilities
- Home health setting with patient visits conducted in the community
- Approximately 6 patient visits per day
- Day shift schedule from 8:30 AM to 5:00 PM
- Working knowledge of Home Care Home Base (HCHB) EMR is preferred
- Main responsibilities include evaluating and treating patients in their homes
- Local candidates within 60 miles are accepted at a reduced rate
- Candidates must provide documentation showing current state license, proof of application, or renewal receipts if licenses/certifications are expiring
Requirements
- Valid CT Physical Therapist (PT) license required; license required if applicable
- Minimum 2 years of physical therapy experience required
- Home care experience is preferred
- Current BLS certification from the American Heart Association required
- Two professional references from prior managers in current discipline/specialty required
