A Home Health Aide provides personal care, rehabilitation, and assists with daily activities for clients at home or in a facility under nurse supervision. They follow care plans developed by clinical staff and support agency goals and policies. Certification or approved training in home health aide services is required for the role.
Responsibilities:
The Home Health Aide is responsible for performing health related tasks and may also provide assistance with personal hygiene, housekeeping and other related supportive tasks to a client in his/her home or facility under the supervision of a registered nurse.
- Carries out the agency’s mission, philosophy, goals and objectives within guidelines of Agency policy and position function.
- Participates in the development and implementation of a realistic goal-oriented plan of care.
- Provides personal care and rehabilitative services and assists in activities of daily living as directed by the Director of Clinical Services or the supervising Registered Nurse.
- Follows care plan instructions as directed by Registered Nurse.
Qualifications:
Current New York Department of Health approved Basic Training in Home Health Aide OR Equivalency Exam or Competency Only Program in Home Health Aide Services
