- Credentialing Timeframe: 4-8 weeks
- Orientation: 2-day classroom HR orientation plus hands-on field training
- Minimum 3 years experience as a Nurse Practitioner (NP)
- Current clinical experience in primary care, preferably with geriatric patients
- Palliative care experience highly preferred
- Home-based care or home health experience strongly preferred
- Active Nurse Practitioner license in North Dakota and Minnesota
- DEA registration in both North Dakota and Minnesota
- Ability to work independently and travel within a 60-mile radius of Fargo
- Deliver primary and palliative care to patients in their homes and assisted living facilities
- Manage a patient panel with a goal of 4-8 visits per day (occasionally up to 12)
- Participate in weekly interdisciplinary team meetings at the office
- Collaborate with the medical director and healthcare team to provide high-quality clinical support
- Document patient encounters and coordinate care plans in compliance with Medicare and state regulations
- Contribute to the growth and innovation of a young, dynamic home-based care program
