Description

Our client is seeking a Case Manager, with experience providing professional intervention, and critical services to clients. Case Manager duties consist of assessment, crisis, intervention and financial assistance.

Responsibilities:

• Conduct comprehensive assessments to identify the social, emotional, and financial needs of clients and their families.
• Advocate for clients by connecting them with appropriate community resources and support services.
• Perform regular home and field visits with clients, adhering to contract requirements and working across multiple assigned sites.
• Maintain accurate and up-to-date client records in both physical files and electronic databases.
• Provide ongoing case management services, including eligibility assessments and service coordination.
• Develop, implement, and monitor individualized service plans to deliver a full range of social service interventions.
• Collaborate with team members and partner agencies to ensure clients receive timely and effective assistance.

 

Qualifications:

• Bachelor’s degree in Human Services, Sociology, Psychology, or a related field (or equivalent experience).
• Minimum of one year of experience in a social services or case management setting.
• Strong organizational, communication, and computer skills.
• Must be able to conduct home and field visits and travel throughout the five boroughs.
• Full-time schedule: Monday – Friday.
