Title: Physical Therapist – Home Health
Location: Medford, OR
Duration: 13 weeks
Shift: Day 5x8-Hour (08:00 – 16:30)
Job Responsibilities and Required/Preferred Skills:
Provide skilled home health physical therapy services to patients in Medford, OR and surrounding areas.
Manage patient caseload of up to 6 visits per day; maintain standard productivity (30 points/day).
Conduct assessments, treatment planning, and interventions across multiple domains:
Orthopedics: Cervicalgia, low back pain, post-op cervical/shoulder/spinal surgery, spinal conditions, total hip/knee replacement.
Neurologic: Cognitive disorders, multiple sclerosis, muscular dystrophy, Parkinson's disease, post-stroke rehabilitation, spinal cord injury, traumatic brain injury.
Professional Skills: Home exercise program, patient/family education.
Assessment Skills: Body mechanics evaluation and rehabilitation progress.
Work across multiple settings: acute care, outpatient clinic, rehabilitation hospital, skilled nursing facility (SNF).
Required Certifications:
Experience Requirements:
Minimum 1 year of skilled home health physical therapy experience.
Oasis experience required for documentation.
First-time travelers not accepted.
