Description

Clinical Pharmacist at Providence Santa Rosa Memorial Hospital in Santa Rosa, CA. This position is per diem and will work 10-hour, Day shifts.

The clinical pharmacist I has the responsibility for the provision of high-quality, individualized, cost-effective pharmaceutical care and integrated pharmacy operational services in collaboration with the healthcare team to optimize patient's medication therapy outcomes and quality of life.

Required Qualifications:

• PharmD - Pharmacy Doctorate or
• Bachelor's Degree in Pharmacy
• California Pharmacist Registered upon hire
• Basic Life Support (BLS) upon hire - where applicable

Preferred Qualifications:

• Acute care hospital pharmacy experience, 1 year or equivalent experience.
• Completion of ASHP-accredited PGY1 residency.

The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
