Position Highlights

• Outpatient-only primary care in a value-based care model

• Patient panel capped at 750 to ensure quality care and manageable workload

• New physicians start with 100 patients and ramp up gradually

• See approximately 18 patients per day

• Monday Friday schedule with one remote/work-from-home day per week

• Limited call responsibilities

Exceptional Clinical Support

• Robust care team including EMTs, APPs, Medical Assistants, and Care Managers

• Dedicated acute care support team with nurses triaging and responding quickly to patient needs

• Strong infrastructure allowing physicians to focus on patient care not paperwork

Ideal Candidate

• Board Certified or Board Eligible in Internal Medicine, Family Medicine, Geriatrics

• Must be bi-lingual in Spanish

• Passion for caring for older adults and practicing in a value-based care environment

• Team-oriented, patient-centered, and quality-driven
