- Additional Info: We are a non-profit healthcare organization with a simple mission: to help older adults live safely and comfortably in their homes and communities for as long as they can, keeping their stays in hospitals and nursing facilities as minimal as possible.DETAILS:- You will participate as a member of the PACE Interdisciplinary Team in the care of high-risk older adults living in the community - Will act as the leader of the Clinical Team whose primary mission is to assist older adults to remain living in the community for as long as it is safe- Monday-Friday scheduling, with a possibility for a 32-hour workweek or 40-hour workweek- Participates in the PACE hospital rounding system as well as nursing home visits- Evaluates the needs of the participants and develops treatment plans in concert with the other members of the Interdisciplinary Team- Experience working directly with Geriatric patients required
