A Health Center in Vermont is seeking a Family Practice physician to provide comprehensive primary care, including patient empowerment, clinical supervision, and care coordination within a Federally Qualified Health Center setting. The role involves participation in quality assurance, disease collaborative efforts, and managing integrated behavioral health programs using EMR systems. Health Center is seeking a Family Practice physician to work at our clinics. -The Center is Federally Qualified Health Centers (FQHC) and provides a network of Family Practitioners, Internists, Psychiatrists, Dentists, and Social Workers. Empower patients to take an active role in health maintenance and promote wellness behaviors Provide appropriate patient follow-up and communicate effectively with patients Refer patients to specialty care as necessary Provide clinical supervision to mid-level providers and nursing staff Participate in quality assurance activities Participate in State and National disease collaborative efforts Community Health Center sites are certified Level III Medical Homes, with a highly developed and supportive panel management and case management system in place. Robust integrated behavioral health program employing Psychiatrists, Behavioral Health Nurse Practitioners and Licensed Clinical Social Workers embedded within its primary care sites. -EMR: Medent.
