Job Description
We are seeking a Board-Certified Nurse Practitioner to join a geriatric-focused care team serving an elderly and frail patient population.
Key Responsibilities:
Perform comprehensive medical assessments
Develop and manage individualized care plans
Evaluate and treat acute illnesses
Participate in discharge planning
Conduct home visits as clinically indicated
Coordinate care across the full continuum of services (PACE model)
Assist with development and review of clinical policies and procedures
Participate in provider on-call rotation
