✅ Requirements
- Active RN license (WA pending accepted)
- Minimum 2 years RN experience required
- BLS required
- Case Management / Care Coordination experience preferred
- Experience with geriatric population strongly preferred
- Comfortable with community-based visits (mileage reimbursed)
- EPIC experience preferred
? Role Overview
- Adult outpatient PACE (Program of All-Inclusive Care for the Elderly) setting
- Care coordination, discharge planning, utilization review
- Manage participants transitioning from acute care and SNF settings
- Interdisciplinary collaboration with physicians and social workers
- Primarily onsite with occasional community/home assessments
