The Opportunity

• Seeking Family Care or Primary Care physician assistants and nurse practitioners.
• Current national certification, DEA, and BLS are required.
• At least one (1) year of primary care experience is required.
• Fingerprinting via DOJ will be required.
• Current CA state license is required.

San Bernardino Health Center - San Bernardino, California

Adelanto Community Health Center - Adelanto, California

Ontario Health Center - Ontario, California

• A Federally Qualified Health Center.
• Referral resources include WIC, Dental, Psych, Nutritionist, and a MediCal case worker.
• Facility hours for this location are Mon-Fri 8am-5pm.
• Good mix of patients including peds.
• 15-18 patients per day per provider.
• Point of Care testing available at site; minimal labs.
