Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Current New York state Physical Therapy license required prior to start. Candidate must meet licensure requirements for any additional states listed in assignment notes if expected to travel across state lines.
• Current CPR certification required.
• Valid driver’s license, active vehicle insurance, and access to a dependable vehicle for patient visits in a home health setting.
• Experience providing physical therapy in home health or community-based settings preferred.
• Ability to complete patient evaluations, develop and document PT plan of care, and submit progress notes per agency policy. Previous experience with home health documentation systems preferred.
• Ability to supervise PTAs and home health aides per state regulations and agency policy preferred.
• Travel assignment experience or willingness to travel to support multiple local locations as part of the assignment preferred.

• Facility Type: Home Health.
• Status: Full Time travel assignment serving Kingston, Lake Katrine and nearby communities.
• Position: Physical Therapist providing evaluation, skilled treatments, plan of care development, progress documentation, and discharge summaries consistent with home health standards.
• Clinical duties include assessing functional status, providing interventions to improve mobility, endurance, pain management, caregiver education, and participation in interdisciplinary communication.
• Must be able to travel on approved assignments to support multiple locations within the service area during the assignment period.
