Home Health Registered Nurse – Days (Contract)

Position Overview:
Seeking an experienced Home Health Registered Nurse for a full-time, day-shift contract. This role requires true home health experience with strong proficiency in OASIS documentation and Homecare Homebase (HCHB). Candidates must live within 30 minutes of the assigned territory or secure nearby housing if traveling.
Key Responsibilities:
- Provide skilled nursing care to home health patients
- Complete all documentation same day using Homecare Homebase at the patient’s home
- Manage an average of 20–25 cases per week
- Maintain productivity standards (average 26–28 points; approximately 5–7 points per day)
- Collaborate with interdisciplinary teams to ensure high-quality patient care
- Remain flexible with patient assignments and scheduling needs
Requirements:
- Active RN license
- BLS certification
- Minimum 2 years of home health RN experience
- Required experience with Homecare Homebase (HCHB) and OASIS
- OASIS-E experience required (post-January 2023)
- No more than a 1-year gap in employment
- Strong time management and documentation skills
- True home health background is an absolute must
Additional Details:
- No on-call requirement
- No overtime permitted
- Extension opportunities may be available
- Mandatory in-office visit once per week (typically Tuesday, Wednesday, or Thursday)
- Branch hours are 8:00 AM–5:00 PM; patient visits may occur outside these hours
- Orientation consists of approximately 2–3 days (modules and team introduction)
- Mileage reimbursement provided
