Work Schedule:
80% FTE, Monday – Friday between the hours of 8:00 AM – 5:00 PM. No holidays or call. Hours may vary based on the needs of the department.

As an Advanced Practice Provider, you will play an integral role in building and expanding a virtual primary care program. You’ll be a part of the ambulatory care teams by providing high-quality, cost-effective care to patients both in person and virtually.
We are seeking a Nurse Practitioner OR Physician Assistant to:
- Be a part of a virtual care team expanding access to primary care through telehealth visits
- Care for the acute and chronic needs of patients from newborn to elderly.
- Perform physical exams and manage the health maintenance needs of our family medicine population.
- Perform routine clinical procedures independently including wart removal, PAP smears and skin lesion removals.
Education:
- If Nurse Practitioner: Master’s or Doctorate’s degree from accredited Nursing Program.
- If Physician Assistant: Bachelor’s or Master’s degree from accredited PA Program.
Work Experience:
Required– Two (2) years of relevant clinical experience
Licenses and Certifications:
Minimum – State of Wisconsin NP or PA licensure
If Nurse Practitioner*:
- Active RN and APRN certifications and licenses, including prescriptive authority, in state(s) of practice AND
- Active board certification as a NP in area of clinical practice. These may include the following: Family.
If Physician Assistant*:
- Active PA license, including prescriptive authority, in state(s) of practice AND
- Active PA board certification
All Nurse Practitioner and Physician Assistant applicants must have*: Active DEA license or ability to apply for such license prior to or upon hire
Qualifications- Master's Degree or Doctorate’s degree from accredited Nursing Program Required
