Position Details:
Location: Lancaster, PA
Setting: Skilled Nursing Facilities (SNF), both short-term and long-term care
Schedule: Monday through Friday, full-time
Flexibility: Autonomy to set your daily rounding schedule
Key Responsibilities:
Provide primary care in post-acute and long-term care settings
Manage chronic and acute conditions, prescribe and monitor medications
Order and interpret lab and diagnostic results
Collaborate with interdisciplinary healthcare teams
Educate patients and caregivers on condition management and healthy lifestyle changes
Build strong relationships with patients, families, and facility staff
Participate in care coordination and promote smooth care transitions
Qualifications:
MD or DO; Board Certified or Eligible in Internal Medicine, Family Medicine, or related primary care field
Active, unrestricted Pennsylvania medical license
DEA registration with prescriptive authority in Pennsylvania
Minimum of 1 year primary care experience required
Post-acute care experience preferred but not required
