BID DUE DATE: 08/07/2025 5pm ESTRATE CAPS FOR SUBMISSIONMalpractice Must be included in rates. Orientation 200 - half the hourly/callback rate kicks in after 8 hours. Per hour oforientation or training beyond 8 hours Hourly 400 - Per hour not OT Holiday 600 - Must be 1.5x the hourly rate. Per holiday hour no OT On Call Daily 3300 - Per 24 hours to include 4 hours gratis24 Hour Holiday Call Rate - 4950 - Per 24 hours to include 4 hours gratisCall Back 400 Per hour ofpatient contact hours after gratis hours are completed gratis prorate to MSAterms.
