This role is for a Per Diem Family Medicine Physician providing primary care services across multiple clinics in the Greater Chicago area. The physician will deliver a range of acute, chronic, and preventive care both in-person and via telehealth, collaborating with multidisciplinary teams in a fast-growing, mission-driven healthcare organization. The position requires board certification, licensure in Illinois, and a minimum of two years’ primary care experience, with benefits including malpractice insurance and continuing education support.

Employment type:
- “Casual Employee” (per diem, hourly clinician role)
- 8-23 patient care hours per week
Location:
- Seeing patients across multiple offices in Illinois
- Weekly schedule will include hours at severalclinics in the Greater Chicago area
What you'll be working on:
- Seeing patients with a broad array of patient needs; conducting a mix of acute, chronic, and well visits (not a panel-building role)
- Treating patients in-office as well as conducting occasional tele-health visits
- Ongoing collaboration with in-office teammates via daily huddles, as well as with virtual clinical teams
- Utilization of your specific clinical training and opportunities to perform in-office procedures
Education, licenses, and experiences required for this role:
- At least 2 years of primary care experience required.
- Currently licensed or ability to obtain licensure in the state of Illinois.
- The ability to build successful relationships with team members and communicate effectively both 1-on-1 and in groups.
- Can thrive in a fast-growing, mission driven organization focused on using data to improve patient outcomes.
- Board Certification.
