A growing post-acute care organization is seeking a compassionate, mission-driven Physician to join its expanding clinical team. This role focuses on providing high-quality primary care to residents in long-term care settings, supporting a healthier, more dignified aging experience.

In this position, you will provide comprehensive primary care to adults and seniors residing in long-term care communities. You ll work in a supportive, physician-focused environment that prioritizes clinical quality, collaboration, and work-life balance.

This is an ideal role for experienced physicians or recent graduates who want to practice meaningful medicine with predictable daytime hours and strong organizational support.

• Conduct admissions, evaluations, routine assessments, and treatment plans

• Manage chronic and acute medical conditions in the LTC environment

• Collaborate closely with interdisciplinary care teams including NPs, nurses, therapists, and facility partners

• Communicate with families and caregivers to support continuity of care

• Document encounters accurately using a provided mobile device

• Provide compassionate, patient-centered care with a focus on dignity and quality of life

• Monday Friday, daytime hours only

• Minimal on-call responsibilities

• No nights, weekends, or holiday rounding
