- Start Date: November 10, 2025
- Contract Duration: 13 weeks
- RN Shifts: Friday, Saturday, Sunday (Blocked Schedule)
- Shift Hours: 12-hour shifts, 3 shifts per week (36 hours/week)
- Estimated Weekly Pay: $2,003 - $2,226
- Minimum 12 months of recent Home Health RN experience (within the last 3 years)
- Proficiency with HCHB (Homecare Homebase) – required
- OASIS documentation experience – required
- Active Wisconsin or Compact Registered Nurse License – required (must be in hand at time of submission)
- Current BLS certification – required
- Deliver skilled nursing care to patients in their homes across the Greater Milwaukee area
- Complete three patient admissions per day and approximately two additional home visits daily
- Utilize HCHB and OASIS for accurate, timely clinical documentation
- Collaborate with interdisciplinary healthcare teams to develop and implement individualized care plans
- Educate patients and families on disease management, medications, and self-care techniques
- Ensure compliance with all regulatory, clinical, and safety standards
