Core Responsibilities:
Patient Assessment
- Collaborate with physicians to establish and evaluate treatment programs, care plans, goals, and discharge plans tailored to patient needs.
- Develop individualized plans of care in conjunction with patients and their families.
- Create realistic discharge plans that incorporate patient and family goals.
Treatment Planning and Provision of Care
- Implement physical therapy treatment plans effectively.
- Select appropriate activities to help patients progress toward their goals.
- Revise treatment goals and plans based on ongoing patient and family input.
- Provide continuous education and training for patients and families.
- Offer discharge instructions, follow-up, and referrals to community resources as needed.
- Serve as a resource for staff, families, and physicians regarding rehabilitation services and equipment.
Documentation
- Document evaluations and care plans according to departmental guidelines.
- Record each treatment session in line with established protocols.
- Complete all required documentation, including family education and discharge summaries.
