Schedule: Monday–Friday, 8:00 AM–4:30 PM
Status: Full-Time
Sinai Hospital is seeking a compassionate and highly skilled Nurse Navigator to support surgical patients through every phase of their care journey. This role is essential in coordinating multidisciplinary care, supporting patients and families, and promoting optimal health outcomes. The Nurse Navigator acts as a central point of contact, advocate, and educator, ensuring each patient receives high-quality, seamless care.
Key ResponsibilitiesDevelop individualized, multidisciplinary plans of care for each patient in collaboration with physicians and the healthcare team.
Provide patient navigation assistance, helping patients access health care and community resources to ensure quality care and appropriate follow-up.
Conduct patient contact through in-person visits in the hospital, Emergency Department, physician offices, patients’ homes, and via telephone.
Advocate for patients, families, and caregivers, addressing needs and supporting policies and programs that promote their well-being.
Complete comprehensive nursing assessments to identify patient and family strengths, needs, and barriers related to disease management.
Ensure responsiveness of agencies and institutions by conducting data collection, assessment, and reporting to support quality improvement and program development.
Collaborate with multidisciplinary teams to design programs and resources for high-risk patients.
Participate in community patient navigation program development and assist with additional duties as assigned.
Required:
Current Registered Nurse (RN) license in the state of Maryland or eligibility for Maryland licensure.
Basic Life Support (BLS) certification.
