Minimum Requirements
- Doctoral, Master’s, or Bachelor's Degree in Physical Therapy from an accredited program recognized by the APTA.
- Active Ohio Physical Therapist license required to start the assignment.
- BLS certification from AHA or ARC may be required.
- Experience in Home Health settings required. (Client-specific requirement.)
- Ability to provide care across the office service area and travel between patient homes. Mileage will be reimbursed between visits from first patient to last patient. (Client-specific requirement.)
- Willingness to follow client dress code: gray or black scrubs. (Client-specific requirement.)
- Minimum education requirement: Doctoral preferred by client; candidates with Master’s or Bachelor’s will be considered if other requirements met. (Client-preferred clarification.)
Assignment Details
- Facility Type: Home Health.
- Shift Name: Days.
- Shift Details: Days 0800-1600. (Client indicates 8:00 a.m. to 4:00 p.m.)
- Weekly Guarantee: 36 hours per week. (Client-specific requirement.)
- Productivity Expectation: 30 points. (Client-specific requirement.)
- On-call/Weekend Requirement: No.
- Coverage Area: Office service area; candidate must travel between patient homes within the service area.
- Type of certification required: Physical Therapist license in Ohio.
