The Pulmonology Nurse Practitioner provides specialized care to patients in skilled nursing and long-term rehabilitation facilities, focusing on pulmonary and acute care alongside other medical specialties. Responsibilities include patient assessment, diagnosis, treatment planning, medication prescribing, care coordination, and patient education within post-acute settings. The role requires an active APRN license, clinical experience, and proficiency with EMR systems to ensure high-quality, interdisciplinary patient management.
TheNextGen Post Acute Care is seeking a compassionate and experienced APRN to join our growing healthcare team, providing high-quality care in skilled nursing and long-term rehabilitation facilities. This role involves diagnosis, treatment, patient management, and interdisciplinary care coordination across a range of specialties, such as pulmonary and acute care, infectious disease, cardiovascular, pain management, psychiatry, and physical rehabilitation.

Job Responsibilities
- Deliver direct patient care in skilled nursing, rehab, and long-term care settings
- Perform health assessments and physical examinations
- Diagnose and develop treatment plans tailored to each patient
- Prescribe medications per Florida guidelines and clinical best practices
- Complete documentation and maintain accurate health records (EMR/EHR)
- Conduct admission and discharge evaluations
- Coordinate care with physicians, facility staff, and specialists
- Promote preventive health practices and counsel patients on healthy living
- Ensure continuity and quality of care with timely interventions
- Participate in facility rounds and contribute to reducing hospital readmissions
- Educate patients and families about treatment plans and care routines
- Comply with infection control standards and regulatory policies
Qualifications
- Active Texas APRN license
- DEA registration (preferred)
- BLS and ACLS certifications
- Active NPI
- Minimum of 3 years of nursing experience (Skilled Nursing preferred)
- Knowledge of post-acute care and geriatric best practices
- Strong communication, assessment, and leadership skills
- Experience with EMR/EHR systems (Point Click Care/ Matrix)
- Comfortable working independently in a fast-paced, clinical environment
Working Hours:
5 Days a week with weekend availability (Pulmonary Specialty)
Skills:

- Performing physical exams and patient observations

- Setting up and monitoring medical equipment
- Communicating test results to patients and their families
- Recording patient medical histories
- Family planning services
