We are seeking a compassionate and skilled Hospice/Palliative Medicine Nurse Practitioner (NP) to join our dedicated team in Ohio. In this vital role, you will provide comprehensive, patient-centered care to individuals with serious, life-limiting illnesses, focusing on improving quality of life and providing support to patients and their families.
Key Responsibilities:
- Conduct comprehensive assessments of patients to determine palliative care needs and goals.
- Develop and implement individualized care plans in collaboration with interdisciplinary teams, including physicians, social workers, and spiritual care providers.
- Manage symptoms related to serious illness, including pain management and emotional support.
- Provide education and counseling to patients and families about illness progression, treatment options, and advance care planning.
- Support patients and families through end-of-life decisions, ensuring respectful and compassionate care.
- Participate in regular team meetings to discuss patient care and improve service delivery.
- Maintain accurate and thorough documentation in accordance with regulatory and institutional policies.
Qualifications:
- Active NP license in the state of Ohio
- 1 to 2 years of Hospice/Palliative Medicine experience (preferred)
- Active DEA
