Our job summary

The Home Health Physical Therapist provides personalized, in-home therapy to help patients regain strength, manage pain, and achieve their rehabilitation goals. Responsibilities include comprehensive patient assessments, treatment plan modifications, documentation, and patient/family education. The role requires a current Physical Therapist license, experience in physical therapy, and preferably home care experience, with benefits including competitive pay, PTO, and continuing education support.

Status: Full-Time
Compensation: $115,000 a year

As a Home Health Physical Therapist, here’s a big-picture view of what you’ll do:

Complete initial and ongoing comprehensive assessments at appropriate time points.
Assess changes in the patient's status since the last visit/treatment day.
Determine whether the planned services should be modified.
Submit documentation of treatment and skilled intervention provided.
Participate in educating the patient and the family, and other caregivers to promote patient progress toward mutually established goals.

A few must-haves for Home Health Physical Therapists:

Current licensure or certification as a Physical Therapist active in Arizona.
One (1) year Physical Therapy experience.
Experience working in home care setting preferred.
CPR Certification (demonstration course required)
Valid Arizona Fingerprint Clearance card.
No Covid Vaccination Required
