Coverage Needed :

• Start Date: As soon as credentialed
• End Date: 6 months from start date
• Schedule: Inpatient & Outpatient; 1 day clinic
• Call: 1:4

Clinical Details :

• Required Procedures: Standard interventional cardiology core procedures
• EMR: EPIC (experience preferred, but training available)
• Support Staff: Medical Assistants and Registered Nurses
• Patient Volume: 10-25 in clinic; 10-50 inpatient
• Assist with both interventional and non invasive

Requirements :

• Will be required to work at 2 sites, one in Indiana, and one in Illinois
• Will need active Medical and Controlled Substance Licenses in BOTH Indiana and Illinois at the time of submission
• Board Certified
• CV must be in Month / Year format
• Must have license in hand at time of submission, or proof that the license application has been submitted
• Any gaps greater than 30 days will require an explanation on the CV
• DEA must remain in state to maintain privileges. Provider must be willing transfer and maintain DEA in state for the duration of privileges being active, or obtain a new in state DEA to remain in state for the duration of privileges being active
